چکیده

rone metabolites in the plasma of these patients. The effect of this is that a commonly used automated aldosterone immunoassay overestimates plasma aldosterone in renal impairment, and the magnitude of this bias increases with the worsening of renal function. In comparison to results for individuals with normal renal function, the assay overestimates aldosterone on average by 129% in hemodialysis patients and 42% in moderate renal failure [eGFR 30–60 mL min 1 (1.73 m) ] and 27% in patients with mild renal impairment [eGFR 60–90 mL min 1 (1.73 m) ]. Overestimation of aldosterone in patients on dialysis and in endstage renal failure is not widely recognized although it has been reported previously (1–3). Our study is the first to demonstrate that this problem extends to mild and moderate renal impairment. Although large differences between immunoassays for aldosterone have previously been described (4), it is likely that direct immunoassays for plasma aldosterone other than the one we used would also be subject to interference from aldosterone metabolites in patients with renal impairment. The screening and diagnosis of diseases such as primary aldosteronism depend on the accuracy of aldosterone measurements. Up to one-fifth of patients with newly diagnosed primary aldosteronism may have significant renal impairment (5) and the assay overestimation we describe may increase the likelihood of incorrect diagnosis at both the screening and confirmatory stages of investigation of such patients. We note, however, that the clinical outcomes reported in the existing literature are primarily based on aldosterone immunoassays. Although we have not confirmed this finding, the requirement of solvent extraction in mass spectrometry would eliminate the overestimation observed. Indeed, aldosterone is not overestimated by mass spectrometry in patients on hemodialysis (2). Our results indicate that caution is needed in the interpretation of plasma aldosterone measurements by immunoassay in patients with renal impairment, and, when accurate measurements are required, a liquid extraction step or mass spectrometry should be considered.

Miniature Pneumatic Curling Rubber Actuator Generating Bidirectional Motion with One Air-Supply Tube

Soft actuators driven by pneumatic pressure are promising actuators for mechanical systems in medical, biological, agriculture, welfare fields and so on, because they can ensure high safety for fragile objects from their low mechanical impedance. In this study, a new rubber pneumatic actuator made from silicone rubber was developed. Composed of one chamber and one air-supply tube, it can genera...

Preanalytical errors introduced by sample-transportation systems: a means to assess them.

Sample transportation is one of the major factors contributing to delays in returning high-quality clinical laboratory results to the patient bedside. Furthermore, sample quality can be compromised by exposure to extremes of temperature and physical forces during transportation. Uncontrolled temperature-induced errors are well understood and are often prevented by using environmentally controll...

Pseudohyperkalaemia with acute leukaemia: association with pneumatic tube transport of blood specimens.

Falsely elevated serum or plasma potassium level can be the result of mechanical injury to blood cells. We describe pseudohyperkalaemia caused by pneumatic tube transport of blood specimens from a patient with leukaemia. Clinicians should be aware of this possibility when interpreting the clinical significance of hyperkalaemia. In leukaemic patients, pneumatic tube transport of blood specimens ...

Hemolysis associated with pneumatic tube system transport for blood samples

OBJECTIVE The frequency of hemolysis of blood samples may be increased by transport in a pneumatic tube system. The purpose of this study was to evaluate the effect of pneumatic tube system transport on hemolysis of blood samples. METHODS Blood samples were transported from the emergency department to the hospital laboratory manually by hospital staff (49 patients) or with a pneumatic tube sy...

Does Pneumatic Tube System Transport Contribute to Hemolysis in ED Blood Samples?

INTRODUCTION Our goal was to determine if the hemolysis among blood samples obtained in an emergency department and then sent to the laboratory in a pneumatic tube system was different from those in samples that were hand-carried. METHODS The hemolysis index is measured on all samples submitted for potassium analysis. We queried our hospital laboratory database system (SunQuest(®)) for potass...
